The squid package has a series of patches in dpatch format, that it applies before building, and removes when cleaning up from a build. Thus attempts to modify the package fall foul if the changes conflict with these patches (and Murphy's law states they will, and they did). A kind soul of the debian-mentors IRC channel pointed me at dpatch-edit-patch, which I think is the right solution. It all becomes much clearer when you know debian/rules is the Makefile and well as a statement of the obvious.
